Appointment Cancellation Policy:
My Simply Smiles requires advance notice of 24 hours to cancel or reschedule any appointment. In the event you are unable to provide us with at least 24 hours notice, we will assess you a $50 no-show fee. My Simply Smiles is happy to store the $50 no-show fee as a credit on your account and this may be redeemed for future rescheduled appointments.
Membership & Package Cancellation & Refund Policy:
My Simply Smiles allows you to cancel a membership purchase within 72 hours for a full refund. You will be refunded the membership cost, minus the full price of your initial appointment, less any membership discounts you may have received. You may cancel a membership purchase after 72 hours for a 50% refund within 14 days. You will be refunded 50% of the membership cost, minus the full price of your initial appointment, less any membership discounts you may have received.
There are no refunds for membership purchases that are cancelled after 14 days from signup.
Product Return Policy
Return Period:
Product returns must be made within 14 days of purchase. Any return requests made outside of 14 days of purchase will not be honored.
Condition of Return:
Products must be in the original, unopened packaging. Opened or used products are not eligible for returns.
Process:
Initiate Return: Contact our customer service team by calling the location studio or by emailing Contact@mysimplysmiles.com within the 14-day period to initiate a return.
Return Authorization: We will provide you a return authorization email with return instructions.
Return to Store: If you are returning the products to the store, we will coordinate a time for you to do so. Hours may vary by day and by location.
Shipping: If you ship the product back to us, we will email you a prepaid shipping label. Customers are responsible for return shipping costs. The cost of the return shipping will be deducted from the amount of your total refund. Refunds will not be issued until we are in possession of your return.
Inspection: Upon receipt, products will be inspected to ensure they meet return conditions.
Refund: If the return is approved, a refund will be issued to the original payment method within 3-5 business days.
Exceptions/Non-Returnable Items:
Products purchased on sale or with a discount are final sale and cannot be returned.
Gift cards are non-refundable.
Exchanges:
We only replace items if they are defective or damaged. If you need an exchange, contact customer service by calling your local studio location or by emailing us at contact@mysimplysmiles.com
Additional Information:
Original shipping fees are non-refundable.
Refunds should only take 3-5 business days, but actual credit may take up to 10 business days to process back to your original form of payment.
Refund Policy
If you are unsatisfied in any way, please email contact@mysimplysmiles.com with your name, appointment time and why you are unsatisfied. We will strive to do our best to make sure you are as satisfied as possible. My Simply Smiles does not guarantee any refunds for our teeth whitening services.
Photo Consent
I grant permission to My Simply Smile for the use of my photograph(s), or electronic media images, in any presentation of any kind and/ or any of My Simply Smile social media outlets. I understand that I may revoke this authorization at any time by notifying My Simply Smile in writing. The revocation will not affect any actions taken before the receipt of this written notification. Images will be stored in a secure location and only authorized staff will have access to them. They will be kept as long as they are relevant and after that time destroyed or archived. I also acknowledge that by tagging My Simply Smile in any social media post that I make, I grant them permission to re-post that photo on Instagram, or to be used in other marketing or social media efforts. I understand that I may revoke this authorization at any time by notifying My Simply Smile in writing. The revocation will not affect any actions taken before the receipt of this written notification.
Payment
I agree to pay for the services that I have signed up to receive from My Simply Smile. By signing, I agree with the pricing for my teeth whitening appointment and agree to pay for these services.
SMS Opt-In
I agree that My Simply Smile can send me SMS messages regarding my appointment as well as coupon codes, sale alerts, and marketing promotions. Carrier rates may apply.
